Because of a new state law, and for the safety of the children, you have to complete a background check and be approved EACH SCHOOL YEAR (running August - August). Click below to download the Volunteer Form. Print and complete "Side A" totally. On "Side B" write name, sign & date where the asterisks are located, and send it back to me. You can fax, scan, email, or send back to the school to my attention, or in the mail at address above.  You can also download the Volunteer Sign Up Form PDF, and let me know what you are interested in doing at the school.
